HART Hub Niagara

Overview

REACH Niagara is proud to be a participating organization in HART Hub Niagara. Launched in Fall 2025, the program provides addiction and mental health treatment for unhoused individuals. HART Hub Niagara also offers supportive services and increased access to supportive housing.

The HART Hub is located on Queenston Street in St. Catharines and is co-located with De dwa da dehs nye>s Aboriginal Health Centre and Quest Community Health Centre. While the Hub office is based in St. Catharines, services are delivered region-wide. Hub service providers travel to support clients within their own communities, meeting people where they are.

Who is the program for?

HART Hub Niagara is funded by the Government of Ontario and was created to specifically address the complex and interconnected needs of people experiencing chronic homelessness across the Niagara region.

What are the benefits?

HART Hub Niagara enhances treatment and recovery outcomes by filling critical service gaps and offering flexible, client-centred care. The model is designed to ensure individuals can access the right supports, in the right place, at the right time. Other benefits include:

  • Addresses identified gaps in treatment and recovery services across the region
  • Region-wide access with mobile services that meet clients where they are
  • Flexible treatment options tailored to individual needs not met by traditional services

How can you access the program?

  • Individuals experiencing homelessness who are not connected to homelessness services (e.g., shelter, street outreach) can call 211 to get support.
  • The HART Hub connects with people through Niagara’s Coordinated Access system to engage those who may benefit from services.

Program Partners

HART Hub Niagara is led by Gateway Residential and Community Support Services Inc, this collaborative effort also includes several other community organizations including REACH Niagara. The HART Hub uses community organizations with expertise in key service areas to provide seamless high quality care throughout the region. Programming is also coordinated with Niagara Health and Niagara Region Homelessness Services to support housing and referral pathways.
