PATH

Overview

One of the largest contributing factors to the growing number of homeless across Ontario is substance use and mental illness. Not all substances are created equal with varying degrees of destructive and addictive side-effects. REACH Niagara’s PATH program is designed to walk with client’s who are ready for change through a physician lead, case manager and peer support worker guided program that has delivered amazing results for our clients. With this program 97% of participants have reported a decrease in the amount of street drugs being used.

The PATH program specializes in helping individuals receive support by supporting them to apply for ODSP and accessing other sources of income. This program is dedicated to working with community partners and completing referrals to other community service organizations to create wrap-around support and collaboration.

Who is this program for?

REACH Niagara’s PATH program is designed for people who are at a high risk of overdose & social factors such as housing status, disengaged from health care and social services, and/or involved in sex work. The largest determining factor of PATH is that the person is ready for change and willing to participate in the program.

Other factors include:

  • 18 years of age and over 
  • At risk of overdose/history of overdoses
  • Substance use disorder
  • May be on methadone or suboxone

How can people access the program?

Not all REACH client’s are able to participate in PATH. It is important that an individual is wanting to participate and engage in the program to aid positive change in their lives.
